The role of cancer-associated fibroblasts in triple-negative breast cancer Triple-negative breast cancer (TNBC) is a breast cancer subtype with a high proliferation rate, a high incidence of metastasis and a poor prognosis It represents some 10-15% of all breast cancers, with an increased prevalence in young women and women with BRCA1 gene mutations Tumours grow rapidly and unlike more slowly growing breast cancer subtypes which have an infiltrative margin, TNBCs tend to have a pushing margin As a consequence, they often present as smooth masses (clinically and on imaging) rather than the more traditional irregular masses more typical of cancer This often lulls clinicians into a false sense of security and tumours are often mis classified as benign, so delaying the diagnosis This difference in the growth pattern of these tumours raised interest in whether tumour stromal interactions are different in these cancers It has been previously shown that in TNBC tumours, a high stromal content is associated with a poor prognosis Fibroblasts, a key component of the tumour microenvironment, are a heterogenous cell type which produce and remodel the extracellular matrix During cancer progression, fibroblasts can switch from a normal cancer-suppressing to a cancer-promoting phenotype, becoming known as cancer-associated fibroblasts (CAF) If and how CAF contribute to TNBC progression remains to be fully clarified in this project We have already undertaken some preliminary work showing differential growth patterns of co cultured TNBC and either normal fibroblasts and fibroblasts derived from TNBC in both 2D co culture or 3D spheroids and have shown very different patterns of growth and variations in the contractile forces of single cells using traction force microscopy The cancer associated fibroblasts were able to promote the growth of TNBC cells in vitro Analysis of the mechanism of this effect is in its early stages but we have shown that nuclear translocation of the mechanosensor, proto-oncogene and transcriptional co-activator YAP may be involved
